I haven’t run into this problem with medicine, but I have about other things. In those cases I handled it by some combination of 1) Explaining that I do care but that I don’t think it’s worth spending resources on expensive otherwise useless signals, and 2) Consciously trying to signal care when its cheap, even though it feels fake and they know it’s a conscious effort.
With medicine, if I actually do care, I’ll research the problem and usually suggest a better treatment. The average doctor is more or less incompetent at what they’re supposed to do (like everyone else), so using him and google as resources is often enough to come up with a better game plan.
