Thoughts on Ray Dalio-style perfect surveillance inside the org? Would that have helped? Basically put everyone on video camera and let everyone inside the org access the footage.

You’d run into cognitive overhead limits. Manually reviewing other people’s conversations can only really happen at 1:1 to 2:1 speeds. Summaries are much more efficient.
Plus, people behave very differently in radically observed environments. Panopticons were designed as part of a punishment system for a reason.
By the time you have an AI that can monitor and figure out what you are actually doing (or trying to do) on your screen, you do not need the person. Ain´t worth the hassle to install cameras that will be useless in 12 months time...
